State issues fines, license actions against 11

The state Department of Commerce and Consumer Affairs and boards and commissions that oversee professions and vocations reported taking action against 11 companies and individuals in November.
The following are the result of hearings or settlement agreements:
The Board of Barbering and Cosmetology has fined Bich Ha Thi Pham, dba Nail Trix in Hilo $500.
It revoked the license of Jeanie S. Bryant of Maui while fining her $1,000 and ordering restitution of $285.
The Board of Medical Examiners has revoked the license of William A. Roberts.
It also has fined David H. Garfield $500.
The Real Estate Commission fined Thomas F. Schmidt, doing business as Tom Schmidt Realtors, $5,000 and revoked his license.
It ordered HHH Investments LLC and Harvey H. Higashi of Maui to pay a $1,000 fine and make restitution of $1,621.35.
The Board of Dental Examiners said Jeffrey A. Burgess will voluntarily surrender a license on Jan. 1.
The Board of Private Detectives and Guards fined L. Siracusa & Associates, Inc., doing business as Phoenix Hawaii, and Louis R. Siracusa, Jr., $2,000 and asked they submited updated employee registration forms.
The Motor Vehicle Repair Industry Board levied a $4,000 administrative assessment against the Hawaii Automotive Retailing Group, Inc., which does business as Saturn of Honolulu. The company is also to make sure each of its vendors is properly licensed by the state.
Maxor National Pharmacy Services Corp. was fined $500 by the Board of Pharmacy.
