What's in it: The name and address of the care home, any violations found and resolutions to those violations. The report is public once the Department of Health accepts any pending plan to correct violations. All resident or witness names will be redacted. Who can get it: General public. Where to get it: Only from the Department of Health's Office of Health Care Assurance in Kapolei. What you need: N/A What it costs: The first 120 pages are free and additional pages are 25 cents each. Contact number: (808) 692-7400 and ask for the nurse consultant. Available online? No. |

| CHILDCARE PROVIDER
What's in it: A Child Care Licensing Record from the state Department of Human Services contains childcare provider information, including abuse and neglect complaints and how they were resolved; a staff listing; a record of staff medical exams, immunizations and employment history; and all building, fire code, food and sanitation inspections. Who can get it: General public. Where to get it: Call the DHS office at 587-5266 for information about childcare providers in the Honolulu and Windward areas. Information about providers in the Leeward and Central O'ahu areas is available from a DHS district office at 675-0470. DHS also works with People Attentive to Children to monitor childcare providers. | RESTAURANT INSPECTION
What's in it: The name and address of the restaurant, its inspection report, any violations found and resolutions to those violations, fines levied. Who can get it: General public. Where to get it: Each island has a branch. If you call ahead of time, branch workers can pull the inspection and let you know how many pages are involved. What you need: N/A What it costs: 50 cents a page. If you send a check, the report can be mailed to you.
