The Hawaii Medical Service Association is launching an online health-risk assessment program that can offer a wellness action plan to target their health risks.

HMSA, the state's largest healthcare insurer, said the online questionnaire was developed by HealthMedia Inc., a health behavior change company.

HMSA members may access questionnaire by going to hmsa.com and clicking on Health Risk Assessment. The questions take about 30 minutes to answer. A health profile will be generated and users will be offered a personal action plan.
