Firms fined over fatal accident

PSC Industrial Outsourcing Co. has been cited and fined $15,000 by the state after a welding accident killed worker Sean Miguel Norva last year at Campbell Industrial Park.

A similar citation has been issued to Pana Corp Inc. for the same accident that occurred on Oct. 7 at PSC — also known as Philips Services Hawaii Ltd., said Jamesner Dumlao, operations manager for the Hawaii Occupational Safety and Health division of the state Department of Labor and Industrial Relations.

Dumlao did not know the amount of the fine or the number of violations for Pana Corp. but said the violations for both companies were considered serious.

"The citations have been issued, and they're going through the administrative process," Dumlao said.

An inspection report for PSC is posted on the U.S. Department of Labor Occupational Safety and Health Administration Web site.

The report lists six serious violations for PSC, each carrying a $2,500 fine.

PSC did not respond to a request for comment.

Dumlao said violations fall into two categories: Serious, which results in death or serious bodily harm, and other than serious.

The maximum fine for a serious violation is $7,000, he said

Norva, 23, was killed when an oil storage tank he was welding exploded at Philips Services Hawaii. Norva was welding a catwalk to the 15-foot-tall tank, which contained waste oil.

Vapors from the tank were ignited and the explosion launched Norva more than 100 feet into a pickup truck parked at an adjacent business.

Norva died of multiple internal injuries. Three other people were injured in the explosion.

A Honolulu Fire Department investigation concluded that Norva inadvertently ignited vapors from the storage tank and classified the incident as an industrial accident.
