Consent forms due today for Hawaii’s Stop Flu at School program

The Hawaii State Department of Health is reminding families of children attending schools participating in the Stop Flu at School program that signed consent forms are due today.

School clinics offering free seasonal influenza vaccinations will begin in October. Parents may contact their child(ren)’s school if they have questions about their school’s clinic schedule.

Forms are available in English and translated versions from the DOH Stop Flu at School Web site are at www.stopfluatschool.com.
