Hawai
i taxpayers will face some changes on state income tax forms as well as national forms this year, according to the state Department of Taxation.

Among the changes to the Hawaii individual income tax forms:

Form N-200V. A new form used to submit tax payments due on Hawaii individual income tax returns. Taxpayers should not attach check or money order directly to the return, but should instead enclose payment with the form and the tax return.

Schedule CR, Schedule of Tax Credits. A new form for use with N-12 and N-15. This schedule is used to consolidate the reporting of all nonrefundable and refundable tax credits, except those reported on Schedule X and the Credit for Child Passenger Restraint Systems. The Schedule CR should be attached immediately after your tax return and before all other schedules and forms.s

New tax credits. On Form N-11, new lines have been added for the following new credits: High-technology Business Investment Tax Credit, Individual Development Account Contribution Tax Credit, Tax Credit for Increasing Research Activities, and the Hotel Construction and Remodeling Tax Credit.

Limited Power of Attorney Check Box. If you paid someone to prepare your tax return for you, answer the question just below you signature that asks, "May the Department of Taxation discuss the return with the preparer shown below?" Checking "yes" allows the department to call the paid preparer if a question about your return arises when it is processed. The limited power of attorney is not effective after a return has been processed.

Change of Mailing Address Check Box. A new check box has been included above the name and address section of the return to indicate a change in mailing address.
